Case Management services are designed to help children up to 18 years old achieve an optimal level of independence by coordinating training in the areas of basic living skills, conflict resolution, problem solving, medication management and socialization. These services are provided in the childs natural environment and include assessments and referrals that link them with a variety of community resources. Children are monitored regularly to ensure an ongoing continuity of care is maintained, newly identified needs are addressed and progress is being achieved toward treatment plan goals.
Case Management Services are community-based and provided throughout Collier County. Youth who access these services must meet certain clinical criteria determined by a clinician through a comprehensive clinical assessment.
